The worker will oversee day to day participant needs, case managing individuals working towards independent living and long-term housing goals and assisting Participants to remain connected to community support workers. The Life Skills Worker will also assist the individual to stay on track with their vocational plan created by the Participant. Service delivery is based on a client centered, nonjudgmental perspective.&nbsp;</div><div><br></div><div><b>Specific Responsibilities</b></div><div><b>•&nbsp; </b>Conduct Individual Case Management meetings with all Participants to ensure compliance with program agreement and mental health and addictions wellness plan.&nbsp;</div><div><b>•&nbsp; </b>Liaise with the Community Living Program Coordinator and applicable team members to ensure a clear and consistent plan is being adhered to.</div><div><b>•&nbsp; </b>Provide individualized support, as needed, to multi-barriered clients in need of low barrier housing and assist them in the development and adherence to a personalized wellness plan.</div><div><b>•&nbsp; </b>Work with the Program Participants to maintain the cleanliness of their suites.&nbsp;</div><div><b>•&nbsp; </b>Encourage and support Program Participants to live as fully and independently as possible within the local community, providing information, emotional, organizational and practical support and training as appropriate.&nbsp;</div><div><b>•&nbsp; </b>Contribute to the protection of Program Participants from abuse, and support Participants when they are in distress.&nbsp;</div><div><b>•&nbsp; </b>Encourage and motivate Program Participants to develop a sense of their own identity, purpose and self-esteem.<b>&nbsp;</b></div><div><b>•&nbsp; </b>Work closely with referral agencies (i.e. CMHA, IHA, Friendship Centre, Brain Injury, etc.)</div><div><b>•&nbsp; </b>Actively seek the views of Program Participants and involve them in decision-making to improve their services and accommodations.&nbsp;</div><div><b>•&nbsp; </b>Monitor emergencies that take place such as medical or mental health assistance and complete necessary incident reports.&nbsp;</div><div><b>•&nbsp; </b>When necessary, be trained to be the first to respond to an overdose situation with on-the-spot overdose response and support.&nbsp;</div><div><b>•&nbsp; </b>Be prepared to work calmly and together with your team, paramedics and police in crisis situations.&nbsp;</div><div><b>•&nbsp; </b>Deliver services to CLP clients not residing at Outlook as needed.&nbsp;</div><div><b>•&nbsp; </b>Ensure compliance with all ASK supportive standards, policies and procedures.</div><div><br></div><div><b>Administrative Responsibilities:</b></div><div><b>•&nbsp; </b>Complete move in/move out paperwork as required by ASK Property Services Department under supervision from Coordinators.&nbsp;</div><div><b>•&nbsp; </b>Maintain complete documentation, statistics and records in files and on ShareVision database.&nbsp;&nbsp;</div><div><b>•&nbsp; </b>Assist team with required verbal and written reports, case notes, letters, memoranda, and year end reports.&nbsp;<b>&nbsp;</b></div><div><b>•&nbsp; The Facilitation Specialist&nbsp;&nbsp;will also provide constructive feedback and coaching to team members who under your supervision deliver on site programs. This role delivers trauma-informed and culturally responsive support to individuals experiencing substance use concerns, mental health challenges, and complex social barriers.</p><p>The primary focus of the Facilitation Specialist is to ensure the consistent delivery of high-quality group programming that complements services provided by the Interior Health Authority (IHA) group facilitator. In addition, the role includes providing individualized case management, life skills development, conducting community-based assessments, and delivering crisis intervention support.</p><p>The Program Facilitation Specialist&nbsp;is responsible for collaboratively developing and maintaining Personal Service Plans with participants to support individualized goal setting, recovery planning, and ongoing progress. The role also requires maintaining accurate and timely documentation, adhering to organizational standards, and upholding strong professional boundaries and ethical practices within a recovery-oriented environment.</p><p><b>Typical Duties Include:</b></p><ul><li>Delivery substance use recovery support curriculum including SMART Recovery, Wellbriety, Art Therapy.&nbsp;</li><li>Create and monitor Personal Service Plans with participants&nbsp;</li><li>Support the Coordinator to assess individuals applying to the recovery program to ensure suitability for recovery success.</li><li>Conduct risk assessments and determine appropriate service pathways, providing mental health support and referrals to internal and external resources as required.</li><li>Attend regular case management meetings to ensure participant’s compliance with program expectations</li><li>Support the Maverick Team to provide individualized support to participants, assisting with recovery stability and discharge planning.</li><li>Support participants in developing life skills necessary for independent living, including financial literacy, daily living skills, and community integration.</li><li>Provide crisis intervention and referrals to appropriate community support when necessary.</li><li>Respond to overdose situations as trained, working collaboratively with team members, paramedics, and police during crisis events.</li><li>Transport participants to appointments and activities as needed.</li><li>Maintain accurate and timely case notes, statistics, and operational documentation in accordance with COA standards and organizational policies.</li><li>Assist with the preparation of verbal and written reports, correspondence, and year-end documentation.</li><li>Participate, when required, in the organization’s Performance and Quality Improvement initiatives.</li><li>Strong understanding of professional boundaries and ethical practice.</li><li>Support daily program operations, including meal service/preparation and maintaining common area cleanliness when required.</li><li>Perform other related duties as required to meet operational and funding requirements.</li></ul><p><b>Qualifications and Requirements:</b></p><ul><li>A combination of a relevant bachelor’s degree (BSW) with extensive experience in a group program delivery/facilitation setting.</li><li>Certification for at least one of the following programs: SMART Recovery, Wellbriety, and/or Art Therapy&nbsp;</li><li>Registered Social Worker (RSW) or Registered Clinical Social Worker (RCSW) is highly desired.</li><li>Strong knowledge of therapeutic group practice</li><li>Understanding of the recovery change process</li><li>Ability to support understanding of all recovery pathways offered at the center: SMART Recovery, Wellbriety, NA/AA, LIFE ring, Dharma,&nbsp;</li><li>Ability to foster the development of the community support network within the chosen pathway</li><li>Demonstrated good judgement, good observation and client assessment skills, tact and empathy in working within a therapeutic recovery environment.</li><li>Knowledge and understanding of Indigenous culture and traditions and a demonstrated ability to work with Indigenous peoples.</li><li>Experience working within a case management model involving multiple community partners.</li><li>Strong ability to recognize and address difficult behaviors.</li><li>Good communication, documentation, interpersonal and conflict management skills.</li><li>Strong working knowledge of community resources.</li><li>Understands and maintains professional boundaries.</li><li>Valid driver’s license and reliable/appropriate transportation with business use coverage.</li><li>Must pass an acceptable criminal record check for the vulnerable sector (Criminal Records Review Program).</li><li>Occupational First Aid Level 1 required.</li><li>Readiness to complete in-house online training within the first month of employment.</li><li>Mental Health First Aid, ASIST, and Trauma Informed Practice an asset.</li></ul><p><i>The ASK Wellness Society is dedicated to promoting diversity/multiculturalism with inclusion as one of our Core Values.
